Memory and medication in Parkinson's disease

The over-arching aim is to explore the effects of two commonly prescribed drugs (which we'll call drug A and drug B) on memory in Parkinson's disease (PD). To be eligible, patients must be community-living, in the early or moderate stages of PD and already be on one of these two drugs. Once enrolled, patient-participants will be randomised to 1 of 2 treatments arms. In each arm, patient-participants will have their memory assessed whilst medicated with each drug. They will also have their memory assessed following a period of medication withdrawal. The only difference between the 2 arms is the order in which the 2 drugs are sequenced (that is in arm 1 drug A is followed by drug B, in arm 2 the order is reversed). The research setting is an easily accessible research centre with easy parking, on the hospital site.

Clearing the Hurdles

ESOS observational study of treatment outcome in early diffuse SSc

Systemic sclerosis (often termed scleroderma) is a rare multi-system connective tissue disease characterised by fibrosis, vascular abnormalities and immune dysfunction. People with the diffuse form of the disease (dcSSc) have a particularly aggressive form and are at high risk of developing lung, kidney and heart involvement with skin thickening spreading from the fingers, feet and face to involve upper arms, thighs or trunk. At present, there is no drug known to favourably influence disease course and both the rarity and severity of early dcSSc make it very difficult to conduct clinical trials. ESOS aims to compare the effectiveness of different immunosuppressant treatments currently favoured by clinicians in the early management of patients with early dcSSc in a prospective observational study, capturing entry and outcome data in a standardised way. Following informed consent, participating patients from centres across the world are followed up at set time-points and treated according to best clinical practice. The data collected will be compared using modern statistical techniques to determine which, if any, is the most effective treatment for early dcSSc.

SAFER 2 trial (Support and Assessment for Falls Emergency Referrals)

This is a pragmatic cluster randomised control trial with qualitative component, involving 25 ambulance stations from ambulance services in London, East Midlands and Wales. Participating stations have been randomly allocated to an intervention group (paramedics assess and refer patients to appropriate community based falls services) or control (deliver patient care as normal). Data are gathered from patients, carers and health services providers about effects on patients and services.
