Title: A Large, Randomised Long-Term Assessment of the Relative Cost-Effectiveness of Surgery for Parkinson's Disease.

Aims: To evaluate the cost-effectiveness of subthalamic nucleus surgery and also the timing of surgery, compared with active medical therapy.

Validated instruments eg Outcome measures

Research project description: An open RCT of early surgery ( STN stimulation or lesioning) versus medical therapy (often using continuous apomorhpine) with surgery deferred for as long as possible. Primary outcome measure is PDQ-39, a patient orientated PD-specific health-related quality of life instrument. Setting will be in neurology and neurosurgical units of major hospitals.

Description of public involvement in research stages: Parkinson's Disease Society (PDS) conditional funding if Medical Research Council peer-reviewed application successful; PDS facilitated discussion at DOH; involvement of PD Nurse specialist network.

Examples of ways the public have made a difference to the research project: PDS support emphasises the focus of the research is on patient reported quality of life in addition to clinical measurements; hopefully, financial and strategic backing will secure MRC (or other) funding.
